Treating the wrong one is the commonest reason patients are disappointed with under eye treatment elsewhere. The assessment matters more here than almost anywhere on the face.

Botox for the Lower Eyelid

One unit, occasionally two, is placed very superficially in the midpupillary line about three millimetres below the lash margin. Relaxing the pretarsal muscle softens the roll when you smile and, in suitable patients, opens the eye aperture by a small amount, giving a more rested look.

This is the most conservative area on the face and it is not suitable for everyone. Patients with poor lower lid tone, significant lower lid bags, or a history of lower lid surgery should not have it, because relaxing the muscle in a lid that already lacks support can worsen puffiness or cause the lid to sit lower. A snap test of lid tone is part of the assessment for exactly this reason.

Tear Trough Filler

Where the problem is a hollow rather than a muscle, a small amount of a firm, low water binding hyaluronic acid is placed deep on the bone in the tear trough to fill the shadow. It is a technically demanding treatment because the skin is the thinnest on the body and the area is rich in vessels. It is done with a blunt cannula, in very small volumes, often across two sessions.

Filler is the wrong treatment if the dark circle is caused by pigmentation rather than shadow, or if there are large fat pads that need surgery. Dr. Ashutosh Shah will tell you which you have.

The Treatment, Step by Step

  1. Assessment of lid tone, the size of any fat pads, the depth of the tear trough and the strength of the muscle roll when smiling.
  2. The correct treatment identified, or a clear explanation of why an injectable is not right for you.
  3. Numbing cream for comfort.
  4. A single Botox unit per side, or cannula placed filler, taking 10 to 20 minutes.
  5. Review at two to four weeks.

Results, Downtime and How Long It Lasts

Botox here shows at 3 to 5 days and lasts about 3 months. Tear trough filler is visible immediately but takes two to four weeks to settle, and lasts 9 to 18 months.

Bruising is more likely around the eye than anywhere else on the face. Small bruises settle within a week. Swelling under the eye after filler can persist for two weeks, so do not judge the result early.

For 48 hours keep your head elevated when sleeping, avoid strenuous exercise, saunas and steam, and do not rub the area.

Risks and Who Should Not Have It

With Botox, the risks are a temporary worsening of under eye puffiness, a rounder looking eye or, rarely, a change in lid position. These are the reason for the strict patient selection described above. With tear trough filler, the risks include prolonged swelling, a bluish tinge if placed too superficially, and, rarely but seriously, vascular occlusion.
